Children's Health and Wellbeing Navigators
We are the Children’s Health and Well-being Navigators for Sittingbourne Primary Care Network.
This service aims to reduce GP appointment times and enable us to have a collaborative working partnership so that together we can provide the very best possible support to patients. We can achieve this through providing:
- Social prescribing
- Care coordination
- Signposting and information for children and their family.
- Empowering families to navigate their child’s care.
We will act as the central point of contact for these patients and a link for supporting services.
Our role is designed to help improve the care and support offered to children, young people and their families, recognising the long- term needs of families and identifying what really matters to them. We can support families/carers where the child has a long term health condition, particularly Neurodevelopmental issue (ie.
