Children's Health and Wellbeing Navigators

We are the Children’s Health and Well-being Navigators for Sittingbourne Primary Care Network.

This service aims to reduce GP appointment times and enable us to have a collaborative working partnership so that together we can provide the very best possible support to patients. We can achieve this through providing:

  • Social prescribing
  • Care coordination
  • Signposting and information for children and their family.
  • Empowering families to navigate their child’s care.

We will act as the central point of contact for these patients and a link for supporting services.

Our role is designed to help improve the care and support offered to children, young people and their families, recognising the long- term needs of families and identifying what really matters to them. We can support families/carers where the child has a long term health condition, particularly Neurodevelopmental issue (ie. ASD and ADHD) or mild anxiety/ mental Health concerns both pre and post diagnosis.

We work with children aged 0 to 18 years and up to 24 yearrs for those with SEND and can take referrals for a wide range of needs, some of which are listed below:

  • Support with finding community/social groups and signposting to online resources and support.
  • Liaising with schools, social workers, Health visitors, Early Help, Community Paediatricians/ CYPMHS and any other professionals.
  • Referrals to CYPMHS, School Health, counselling, NELFT etc. As well as, referrals for children with behavioural difficulties, SEND, health concerns, ADHD & ASC pathway for diagnosis
  • Support with paperwork, chase referrals etc.
  • A child or family that requires multiple agency care for care coordination

Appointments can be made with the Children's Navigators via booking into an Initial slot on the Swale Clinical Services Hub.
